Scout Permission Form
Scouts going on a field trip stated in the class description, will need a completed and signed Permission Slip.
PLEASE NOTE – Scouts will not be allowed to participate in field trips without a completed Permission Slip.
NO REFUNDS will be processed for activities missed due to a missing Permission Slip.
Required Scout Health Form
The museum has partnered with CampDoc to provide an online health information management tool for summer campers at the Houston Museum of Natural Science. Registered patrons should receive an email from noreply@campdoc.com with the subject: Houston Museum of Natural Science via CampDoc. Please accept the invite and complete the profile for your registered camper or scout.

Health & Safety
The health and safety of our patrons is always our first priority. The Summer Camp at HMNS and Summer Scouts programs have extensive health and safety procedures and staff training in place every year.

Accessibility
We are happy to accommodate children with disabilities and special needs. Please notify the Summer Camp Director of your child’s disabilities and special needs PRIOR to your child’s first day in the program.
If you have a camper who may need assistance to complete activities, a parent or caregiver should expect to stay in the room to assist. The individual that accompanies the camper must pass a background check. Please contact the Summer Camp Director at 713-639-4651 to discuss any special needs to help your child be successful at the museum.
